i take the ages given with a grain of salt and it hints at that i think. i take geographical things like the size of westeros with a grain of salt too.
also ygritte's story to jon about the wildling raider whose bastard with a stark daughter became the heir of winterfell kind of illustrates that it's not a neat lineage, they just keep the name going as an institution. and lots of houses have died out and new ones begin in the books.
