bibas i need to write a leave of absence request letter for grad school and idk what to put in it

my prof said it didn't need to be too personal or anything, just like "due to personal/family reasons i have to leave the program" and then just say when i think i might come back (even if im not totally sure yet)

is it okay if it's just a couple of sentences long tho?
