depends on the state, some states have in the past taken a trans child into foster care specifically so they can get medical care but it's far from standard. what state is this?
nayrt but BC is the last province in canada to make any trans-specific legislation on protective statuses and shit, but i wouldn't say its entirely impossible that your niece would get help from child services in foster care
i would call them or maybe a lawyer who specializes in this and ask, also any trans organizations in your area that could give advice because they would know legal matters like this
